As HIE’s ramp-up capabilities, calculation of Clinical Quality Measures (CQMs) is a hot topic.

It’s browser-based dashboard provides an at-a-glance snapshot of progress and the drill-down is an outstanding troubleshooting tool.
Digital Quality Measures (dCMs) are an area where Dynamic Health IT combines our CQM and FHIR expertise.
Patient Portal
The Our ConnectEHR Patient Portal enables patients to view, download, and transmit health information:

Patients can view their data (potentially across multiple locations) at one website.

Data can be aggregated from multiple hospital and provider settings, giving patients a longitudinal view of their medical and wellness history.
CCDA
Some HIE’s may not be able to support the latest CCDA requirements (2.1) or the various types of CCDAs. We can help in this area by converting between various CCDA versions and other representations of data from the Common Clinical Dataset and US Core Data for Interoperability (USCDI):

On the receiving side, our Dynamic FHIR API parses all required CCDS sections of a 2.1 CCDA and makes the data available as FHIR resources. New USCDI elements are currently being added.

On the sending side, we can accept data in many formats to build a 2.1 CCDA.
Dynamic FHIR
Dynamic Health IT also supports two versions of Electronic Case Reporting (eCR):

We generate the Electronic Case Reporting 2.1 CDA document.

Using FHIR, we can send to the CDC’s new SMART on FHIR app called “eCR Now”, which uses FHIR DSTU3 to trigger automatic reporting without disruption of provider workflow when a diagnosis or problem list item is recorded.
