Admission, Discharge, and Transfer (ADT) Event Provider Notifications
CMS Conditions of Participation (CoPs) requires hospitals, including psychiatric hospitals and CAHs, to send electronic patient event notifications of admission, discharge, and/or transfer to another healthcare facility or to another community provider or practitioner. This improves care coordination by allowing a receiving provider or practitioner to reach out to the patient and deliver appropriate follow-up care in a timely manner. Dynamic Health IT is eager to help you comply with the CMS regulation for ADT Notifications. Hospitals are required to have a solution in place or risk losing Medicare and Medicaid reimbursement dollars. Our ADT solution, TIE, monitors your HL7 ADT interface and sends admission, discharge and transfer notifications to appropriate PCP and care managers.

In conjunction with our partner, MaxMD, TIE sends alerts directly to the provider’s 2015 Edition Cures Update Certified EHR (CEHRT) using Direct protocol and the providers’ published Direct addresses through the recipients' existing infrastructure. TIE converts admission/discharge and results into an acceptable format, delivering critical care information seamlessly into provider and care team workflows – no provider portal is required.
